Objective: Determine the incidence of non-alcoholic fatty liver in metabolic syndromes diagnosed with abdominal sonography and FLI (Fatty Liver Index) in the HRUJMCB in the period September-December 2023. Methods and techniques: A descriptive correlational, cross-sectional, primary source study was carried out. A collection instrument was used that consisted of a questionnaire with 14 questions combined with blood tests and abdominal sonography to assess the criteria for MS and identify those who suffered from fatty liver. In the statistical analysis, chi square was used to associate two qualitative variables such as smoking and sex. Results: Regarding the results of the fatty liver index (FLI), 21.3% (n=22) had a low probability of having fatty liver when related to the sonographic findings, 90% (n=22) did not have data on hepatic steatosis, by sonography and only the remaining 9.1% had grade I hepatic steatosis. The 31.1% (n=32) who were in the intermediate zone of which 71.9% (n=23) did not have sonographic data of steatosis, while in 18.8% (n=6) reported grade I steatosis and 9.4% (n=3) grade II steatosis. The remaining 47.6% (n=49) had a high probability of suffering from fatty liver according to the FLI, of which in 31.7% (n=17) hepatic steatosis was not evident by sonography, in 28.6% (n=14) Grade I steatosis was reported, in 30.6% (n=15) grade II steatosis and in 6.1% (n=3) grade III hepatic steatosis. The P value was 0.000, giving it statistical significance, since in the low and intermediate probability groups the IFL showed sensitivity for the diagnosis. However, in the high probability group it can be seen that 34% of the patients had no sonographic data of steatosis, so the index did not show good sensitivity in this group, since it had a tendency to overestimate the diagnosis.
